30 year male patient presenting with acute neck pain radiating to right upper extremity, the pain is associated with weakness and numbness in the C4-C5 and C5-C6 dermatomal distribution.

He has been having pain for 6 weeks with no improvement on PO analgesics and rest. He tried physical therapy with no improvement also.

His XR of the Cervical spine preop reveal C4-C5 and C5-C6 Degenarative disc disease at the corresponding levels with large disc herniation at C5-C6 and over the right side and small disc herniation at C4-C5 same side.

The patient underwent an anterior cervical diskectomy and decompression with fusion using 2 cages.

The stand alone Divergence® cages incorporate two screws each going in a cephalad and caudal angle fixing the cage in place.

The cages were filled with bone graft before implantation.

There was neuromonitoring during the case in order to ensure complete safety especially while working near critical nervous stuctures. We also used surgical loupes for decompression and we used a small 2cm incicion to perform our Smith-Robinson approach.

The procedure was 3 hour long and at the conclusion of the procedure there was adequate foraminal deompression with removal of all the disc tissue at C4-C5 and C5-C6.

The patient had resolution of his neck pain and right upper extremity pain directly after the procedure and was ambulating the second day with a cervical neck collar in place.

He was discharged from the hospital the second day after surgery and is faring well
